About the Role: LVN/LPN Care Management Specialist

As a Remote LVN/LPN Care Management Specialist, you'll be an integral part of our Care Management team, focusing on the reputed company needs of our dual-eligible population. Under the guidance of a Registered/Licensed RN, you'll coordinate care for members experiencing significant health changes, ensuring seamless transitions between care settings. Key Responsibilities:

• Facilitate Interdisciplinary Care Team Meetings with Social Services, Care Management, PCPs, and other key stakeholders to discuss service needs and support safe transitions.

Essential Qualifications:

• 2+ years of LPN or LVN nursing experience, with a strong background in care coordination or a reputed company field.
