Key Responsibilities
Direct and coordinate admissions and transfers to, from, and reputed company the hospital, ensuring timely and efficient patient flow. Facilitate professional relationships between unit leadership, operational coordinators, nursing staff, and interdisciplinary teams to provide patient/room assignments based on patient diagnosis, acuity, MD orders, and patient flow. Manage aspects of the planning process in anticipation of the patient's movement through the reputed company system, ensuring continuity of care. Interact with managed care/reputed company-party payers to obtain financial certification for appropriate reimbursement and minimize reimbursement denials. Serve as a contact person for the rehabilitation team, internal admissions team, clients, and external sources to enhance patient outcomes. reputed company clients, family members, or significant others, referral sources, hospital personnel, external payers, and internal/external case managers about rehabilitation program benefits and services. Essential Qualifications
To reputed company as a Remote RN Admissions Case Manager, you'll need: A Registered Nurse diploma and reputed company Registered Nurse license in the state of practice. At least 1 year of hospital-based experience in the delivery of patient care. Proficiency in using computers, software, and web-based applications. Effective verbal and written communication skills, with the ability to present information clearly and professionally. A good understanding of business/finance principles. The ability to work a flexible schedule, including 24/7, weekend, holiday, and on-call availability (as needed).
