Family Nurse Practitioner (FNP) - Santa Ynez Valley
Hope Valley Health Services, a primary care clinic proudly serving the Santa Ynez Valley community in Buellton, California, is seeking a Family Nurse Practitioner (FNP) to join our growing team. This role offers a flexible, patient-centered practice environment with a current schedule of three full days on-site per week and an optional fourth day of telehealth. As our patient base continues to expand, this position is expected to transition into a full-time role. You will work alongside our physicians and clinical staff to deliver high-quality primary care services to patients of all ages in a collaborative and supportive setting. Buellton is located in the heart of California's Central Coast wine country, offering an exceptional quality of life with a small-town atmosphere, excellent schools, and easy access to Santa Barbara, Los Olivos, and Solvang. - Provide comprehensive primary care services including health assessments, preventive care, and management of acute and chronic conditions
- Perform physical exams, order and interpret diagnostic tests, and develop individualized treatment plans
- Prescribe medications and therapies in accordance with California NP scope of practice
- Deliver telehealth consultations as required
- Educate patients and families on wellness, disease prevention, and treatment options
- Coordinate care with physicians, specialists, and other members of the clinical team
- Maintain accurate and timely documentation in the electronic health record system
Qualifications:
- Master's or Doctoral degree in Nursing (MSN or DNP) from an accredited program
- National board certification as a Family Nurse Practitioner (FNP-BC or FNP-C)
- Active or eligible for California Registered Nurse (RN) and Nurse Practitioner license
- Active or eligible for DEA registration and furnishing number
