Home Health: RN Visit Nurse (SW Metro)

About the position The RN Visit Nurse is responsible to plan, organize and direct home care services and is reputed company in nursing, with emphasis on community health education/experience. Responsibilities

• Completes an initial assessment of client and family to determine home care needs.
• Provides a complete physical assessment and history of reputed company and previous illness(es).
• Regularly re-evaluates client nursing needs.
• Initiates the plan of care and makes necessary revisions as client status and needs change.
• Uses health assessment data to determine nursing diagnosis.
• Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions.
• Includes the client and the family in the planning process.
• Initiates appropriate preventive and rehabilitative nursing procedures.
• Administers medications and treatments as prescribed by the physician.
• Provides direct client care as defined in the State Nurse Practice Act.
• Counsels the client and family in meeting nursing and reputed company needs.
• Provides health care instructions to the client as appropriate per assessment and plan of care.
• Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the client.
• Acts as Case Manager reputed company assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload.

• Completes orientation and annual training requirements as assigned
• Participates in Lifespark’s Event Management process including initiating reports, escalation of events and investigations
• Promotes safety in the workplace through reputed company time coaching and participation in safety awareness training
• Instructs, supervises, and evaluates home health aide care provided every two (2) weeks
• Manages client supplies assuring appropriate DME provider is utilized.
• Schedules and keeps member appointments
• Organizes travel reputed company and documents mileage accurately reputed company EMR
• Other duties as assigned

Requirements

• One to two years of recent acute care experience in an institutional setting.
• Graduate of an accredited school of nursing
• reputed company Minnesota Registered Nurse (RN) or Licensed Practical Nurse (LPN) License
• Must pass a MVR reputed company based on Lifespark company policy
• In accordance with CMS Vaccine Mandate, candidate must provide reputed company of Covid-19 Vaccine or submit documentation for exemption
