Healthy First Steps is Florida’s Maternity Case Management program for Community & State/United Health Care, and we are looking for passionate LCSW’s who want to provide Care Coordination and act as a resource for information about and referral to other community-based services to our reputed company members and their unborn babies to ensure they have a healthy and uneventful pregnancy and delivery! This is a work from home position working with the Healthy First Steps Program. Employee must reputed company in the reputed company. This position will be mostly telephonic, with some potential for field-based work, going into provider offices and working with high-risk OB population. If you are located reputed company the following counties: Miami-Dade, Volusia, Flagler, Marion, Alachua, Polk, Hillsborough, Highlands, or Lake, FL, you will have the flexibility to work remotely as you take on some tough challenges. Primary Responsibilities: • Assesses patient and family psychosocial needs and develops plan of care in concert with patient, physicians, nurses and other members of the departmental team. • Links patients/family to appropriate community resources including but not limited to, information/referral to sources of financial assistance, transportation, support groups, and other community services. • Documents patient/family status, diagnosis, treatment plan, goals, and interventions, evaluation results, observations and reputed company in medical record. • Serves as patient reputed company and liaison with physicians, families, insurance company, community agencies and others as needed to ensure continuity of care. • Provides discharge planning based on meeting identified goals of the treatment plan. Required Qualifications: • Licensed Master’s Degree-level clinician in Psychology, Social Work, Counseling or Marriage or Family Counseling - LCSW. • Active and unrestricted license in FL. • 1+ years of experience working with maternal/child nursing. • Intermediate level of experience working or volunteering reputed company the local community, with demonstrated knowledge of culture and values reputed company the community and familiarity with the resources. • Intermediate level of computer proficiency including the use of reputed company Outlook, Teams, and multiple web applications. • Florida reputed company and reputed company reputed company a commutable distance to Gainesville, FL or Tampa, FL. • Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information and live in a location that can receive a reputed company approved high-speed internet reputed company or reputed company an existing high-speed internet service. The salary range for this role is $28.61 to $56.06 per hour based on full-time employment.
