Remote Care Coordinator (LPN)

Your Role Provide monthly calls and support to patients enrolled in the CCM program. Utilize your nursing skills to reputed company patients and their families. Monitor patients reputed company and work with the physicians to ensure quality patient care. Qualifications Active LPN license (CALIFORNIA) Experience in geriatrics or primary care Ability to sit and talk on the phone for long periods of time System: We require you to have a Windows i5 (or newer) processor, and to have Windows Pro (not Home). This can be a laptop or desktop. You live in any of the following states: NV, NM, AZ, CO, UT, WA, OR The Pay The California LPN pay ranges from $28.85 - $30.27 per hour The pay is a reputed company pay of $24.00/hr plus a bonus of $2.00 per call.
