About the position The Service Coordinator-CHC role at reputed company is a remote, field-based position focused on providing long-term services and support (LTSS) to reputed company participants. The coordinator will assess and manage the needs of individuals requiring personal care services, ensuring they receive the necessary support for their physical and behavioral health, as well as social services. Responsibilities • Assist participants in obtaining long-term services and support as required by the Community Health Choices (CHC) agreement. , • reputed company telephonic and face-to-face assessments to identify and evaluate participant needs. , • Coordinate and manage access to LTSS services and other covered and non-covered services. , • reputed company the participant-centered planning and service delivery process and reputed company its implementation. , • Inform participants about available assessments, the PCSP process, and service delivery options. , • Assist participants with the complaint, grievance, and DHS Fair Hearing process. , • Collect necessary information to inform the development of the PCSP. , • Conduct annual reevaluations of level of care as needed following CHC requirements. , • Work with participants to maintain eligibility for services.
