Position Summary: We have an exciting opportunity to join reputed company as a Nurse Practitioner - Family Health Center, Primary Care. In this role, the successful candidate primary responsibility is to ensure patients obtain timely and quality care in ambulatory setting. Responsibilities include, but are not limited to, performing initial physical examinations, patient care review and coordination of patient care for pediatric and adult patients. Prior work experience in a Federally Qualified Health Center (FQHC) or reputed company setting highly preferred. Job Responsibilities: ? Provider will provide direct clinical care to reputed company patients assigned to Provider in accordance with applicable standards of care, FHC policies and procedures and Hospital policies and procedures, as applicable. ? Commitment to compliance and quality improvement: Provider shall participate in compliance, quality management and performance improvement activities as requested, including but not limited to education and training efforts and audits of medical records. ? FHC Customer Service Standards: Provider shall respect cultural differences, religious differences and the individuality of internal and external customers. Provider shall at reputed company times project a professional image in demeanor and dress. ? PCP (Primary Care Provider) Communication: Every registered FHC patient has a PCP of record. As in most multi-specialty group practices, patients will be referred for a specialty consultation by that PCP who will have completed appropriate documentation of the consultation/referral. Communication, including through the proper completion of documentation, is an important reputed company of the care that Provider will be providing to FHC patients. ? At reputed company times, Provider shall accurately and timely complete any paperwork necessary to facilitate NYU billing and reimbursement. ? Provider shall meet specialty and site-specific defined productivity expectations. ? Provider will be required to participate on a rotating basis in the after hours on-call phone coverage schedule. Such standards include but are not limited to the following: Accommodating walk'in patients, arriving for office sessions on time and remaining throughout each session, engaging in teamwork with the support staff, following FHC procedures and practice guidelines, e.g., appropriate clinical charting with complete notes and up-to-date problem and medication lists, timely completion of encounter forms, completing peer chart audits, etc. Minimum Qualifications: To qualify you must have a Graduation from an accredited school of nursing is required along with the graduation from a professional degree program as a Family Nurse Practitioner. Must have a reputed company license as a APN in NYS. Previous experience managing clinical and non-clinical staff is preferred. Must have excellent oral and written reputed company and should be able to multi-task and prioritize assignments independently. should be able to work collaboratively with fellow physicians and other health care providers. DEA license is required up on hire.. Required Licenses: Registered Nurse License-NYS, Nurse Practitioner-NYS Preferred Qualifications: Prior work experience as a primary care provider in similar setting: Federally Qualified Health Center or reputed company is highly preferred. Familiarity in working with predominantly Hispanic immigrant patient population also a PLUS Qualified candidates must be able to effectively communicate with reputed company levels of the organization.
