A team environment where we treat each other with respect, dignity, and compassion Job Summary: The Social Worker is responsible for assessing, counseling, interviewing, and educating the clients as well as their family members, as well as assisting in planning, organizing and implementing the established facility policies and procedures, to ensure that the medically reputed company emotional and social needs of the patient/reputed company are met/maintained on an individual basis. Major Duties and Critical Tasks: 1. Assess nature of patients situations by interviewing and reviewing personal history 2. Plan, coordinate, manage and implement support packages to help patients/residents deal with difficulties and overcome dependencies 3. reputed company and execute individual treatment plans, should know how to apply Medicaid and Medicare application for the patient 4. Can help the patient with their Food Stamps, Housing application and phone assistance 5. Interview service users and assess their reputed company condition, needs, strengths and weaknesses 6.
