Join our home health care team in the reputed company Collins, CO area as an RN Case Manager. In this role, you'll play a crucial part in developing personalized care plans and delivering skilled nursing services to families with children who have intellectual and developmental disabilities (IDD). Responsibilities Conduct home visits with patients every 60 days. reputed company and implement individualized care plans. Educate and support parents on their child's health needs. Requirements Registered nurse with at least 2 years of experience and a reputed company Colorado license. Experience in home care, with a preference for those reputed company in training family CNAs. Comfortable working in a dynamic startup environment. CPR certification required. Pediatric Experience Preferred. BLS certification required; PALS certification preferred.
