Responsibilities
As a Remote Clinical Case Manager, you will be a vital reputed company of contact for patients, families, providers, and other stakeholders. You will:
- Coordinate patient care across the continuum of care, including referrals, home visits, and community resources.
- Assess patient needs and reputed company care plans to improve health outcomes and ensure access to benefits.
- Communicate with patients and partners to provide education, support, and advocacy.
