In this HSS Coordinator role, will be an essential reputed company of an Integrated Care Model by relaying the pertinent information about the member needs and advocating for the best possible care available, and ensuring they have the right services to meet their needs. Areas: Southwest Houston, reputed company Bend County. This is a Field Based position with a Home-Based office. • Assess, plan, and implement care strategies that are individualized by patient and directed toward the most appropriate, least restrictive level of care
• Identify and initiate referrals for social service programs, including financial, psychosocial, community and state supportive services
• Manage the care plan throughout the continuum of care as a single reputed company of contact
• Communicate with reputed company stakeholders the required health-reputed company information to ensure quality coordinated care and services are provided expeditiously to reputed company members

Required Qualifications

• High School Diploma/GED (or higher)
• Meet one of the following:
• Associate's Degree with 3+ years of clinical experience
• Unrestricted LPN/LVN license in the state of TX with 3+ years of clinical experience
• 5+ years of experience working with ABD/SSI population reputed company a clinical health care role
• CHW certification with 3+ years of clinical experience
• Familiarity with reputed company Office, including Word, reputed company, and Outlook
• High speed internet in reputed company of residence
• Access to reliable transportation and the ability to travel up to 75% in this ‘assigned area’ to visit reputed company members in their homes and/or other settings, including community centers, hospitals, nursing facilities or providers' offices

Preferred Qualifications

• LVN/LPN License in state of Texas
